School and Sports Physical Requirements: Please Read Carefully
The School Health Law requires medical examinations for children in grades K, 6,11 and new students from out of state. These grades were selected because they represent critical periods of growth and development in a child's life. If your child will be in one of these grades next school year, we are recommending that theses examinations be done by your family physician since he can best evaluate your child's health and assist you in obtaining necessary treatment and corrections. Please understand this examination would be done at your own expense.
If you desire to have the examination done by your family physician, please have him complete the private physical exam form and return to the school no later than September 15th.
Private exam forms will be given to every student the year prior to the year required. Forms may also be obtained in any school health room. Please contact your child's school nurse if you have any questions.
Arrangements can be made through the School nurse for the School Physician to complete this requirement. School physical examinations are to be scheduled with the attending physician by the school nurse. Parental written permission and a recent health history must be obtained prior to the exam.
SPORT PHYSICAL EXAMINATIONS:
Recertification for winter and spring sports MAY be done on students who had their original sport physical exam by the school physician during the fall and/or winter exams. Private sport exams can not be recertified by the school physician.
All forms are available with the coaches, the Middle School and the High School Offices.
